This article in Health Affairs highlights the work of NH&RA member Beacon Communities and the Richmond Health and Wellness Program (RHWP). An interprofessional group of health care students and faculty members provide services to low-income, senior residents of Beacon Communities’ Dominion Place building. The nurse-led RHWP team serves as an interface between the patient and the primary care physician and provides health screenings, chronic disease management advice, nutrition advice and referral appointments. The program is built around what the residents have said they want, and its goal is to support their aging in place—that is, helping residents remain part of their apartment building community and avoid, if at all possible, the nursing home.
